In this section you will find the current Athletic Participation Form (physical form) and the Concussion Form. According to new federal legislation, Gfeller-Waller Law, both athlete and parent/guardion must fill out the concussion form prior to trying out for a sport.  

The concussion forms must be signed by the student and the parent before a child can participate.

Both forms must be completely filled out and returned prior to trying out for an athletic team. all forms are on the same form now.

The participation form must be filled out by a doctor prior to try-outs.

Eligibility requirements are also listed in this area. Wake County is responsible for eligibility requirements. The school is unable to change any regulations in regard to eligibility.
