Welcome to the High School Nurse's Office

Free or low cost health insurance is now available for all children and teens in New York through Medicaid and Child Health Plus.  Many uninsured parents and other adults may also be eligible for Medicaid and Child Health Plus.
 
If you are interested and would like to meet with a facilitated enroller to complete an application you will need the following proof:
 
Age
Identity
Home Address
Income
Citizenship or immigration status
Adults may need to provide some additional documents
 
Please contact the school nurse if you would like to have a facilitated enroller contact you. A form will be provided to complete, sign and date giving the faiclitated enroller permission to contact you.
