Physical Examination for Participation in Sports |
Sport Physicals are given by our school physician, Michael Meese, M.D., several times during the year free of charge:
The actual date will be announced to the students several weeks before. A packet including a pre-participation questionaire and exam form will be handed out. This packet is due back one week before the exam date to allow for pre-screening and scheduling of the students. Forms due by June 1, 2012
Physicals maybe performed by the student's personal physician. However they must be documented using the same packet and will be reviewed by our school physician before participation in practices and games begin.
Once the pre-participation questionaire is reviewed and the physical exam is completed. All students will be given a medical inspector's certificate card which they should show to their parents and to bring to the first day of practice.
