Physical Examination for Registration Form is required before all new students begin school. The physical examination must have been performed within 365 days of entrance into school
Sport Physical Forms are required before all middle school students begin athletic participation. All physical exams performed by the student's personal physician must be given on or after June 1st, documented on the form below and reviewed by the school physician before participation in practices or games can begin.
Administration of Medication Form is required before any medication can be taken at school including non-prescription medication such as Tylenol. All prescribed medication must be delivered to school nurse by the parent or guardian in the original, labeled container. Self-administration means carrying and taking medication without intervention of the school nurse and restricted to students with asthma and other life-threatening illnesses.
