The Physical Examination for Registration form is required before any child can begin school. The physical must have been performed within 365 days of entrance into school.
The Authorization to Administer Medication forms are required before any medication can be administered in school, including over-the counter medicatons like Tylenol or Motrin. Medication must be delivered by a parent or guardian in an unopened original container.
Self-administration is restricted to students with Asthma or to students who are in danger of having an anaphylactic reaction.
Students with Asthma must have the Asthma Care Plan completed by their physician and Students with Food Allergies must have their physician complete the Food Allergy Care Plan.
