Overbrook Parent Permission Form for Counseling (DOC 1.20 MB)
This form is for parents of Overbrook students to sign to give consent for their child(ren) to participate in individual counseling sessions.

Overbrook Release of Information Form (DOC 28 KB)
This form is for parents of Overbrook students to sign to authorize the counselor to speak with outside sources, such as other counselors, doctors, tutors, etc.

St. Cecilia Parent Permission Form for Counseling (DOC 80 KB)
This form is for parents of St. Cecilia students to sign to give consent for their daughter to participate in individual counseling sessions.

St. Cecilia Release of Information Form (DOC 28 KB)
This form is for parents of St. Cecilia students to sign to give approval for the counselor to share information with outside sources, such as other counselors, doctors, tutors, etc.
